REScure health care Services Private Limited (“REScure health care”) is a integrated healthcare provider with a unique model enabling delivery of the complete spectrum of healthcare services utilising its virtual platform and/or the mobility platform (“REScure health Platform”). The various health and wellness services offered through REScure health Platform include but not limited to Nursing care at home, general Care at Home, Diagnositc services at home (Lab tests) Pharmacy at Home (Home Delivery of Medicines and Medical Supplies), Elder care at home, Consultations at home, transportation and such other services as may be provided from time to time (“Services”). Certain of these Services are offered by third party healthcare and wellness service providers who are associated with REScure health care (“REScure health care Associate”). In such cases, REScure health care merely facilitates the interaction between users/customers and REScure health care Associate through the REScure health Platform. The Services offered through REScure health Platform are subject to the following terms and conditions.

1.Services offered through home health care Platform are subject to the terms of our Website policies, [i.e. Terms of Use, Privacy Policy, Services Rule Engine Policy and Refund Policy etc.] (“Policies”) available at www.rescurehealthcare.com and annexed hereto. By contacting REScure health care for the Services or availing the Services or by registering with us, now or in the future, you signify that you agree to these Terms of Registration (“Terms”) and the Policies.

2. Certain of the Services offered from REScure home health care, specific diagnostic services, nursing Care at home) Elder care may require specific consent from you. You will not be provided such Services unless you have provided your specific consent. The Services provided by REScure health care or its Associates are subject to these general terms of Registration and the terms of Specific Consent for certain services offered through REScure health Platform.

3. The Policies are subject to modifications, at the discretion of REScure health care. Further, REScure health care may from time to time formulate new policies for availing all or any of the Services. Changes to the Policies or addition of new policies shall be notified on the Website. REScure health care will also intimate the changes to the Policies, through e-mail, SMS and/or post, as per the details provided by you. Please make sure that you provide a valid email-id, mobile number and contact information and you update it as and when there is a change. You understand that your use of the Services after any changes to the Policies or addition of new policies constitutes your acceptance to be bound by the most recent version of the Policies. If the Policies (including changes and additions thereto) are not acceptable to you, you have the absolute right to terminate your use of the Services.

4. Persons below the age of 18 and otherwise incompetent to enter into a contract as per applicable laws cannot avail the Services. Such persons can avail the Services if they are represented by their legal guardian, who shall be subject to the Terms.

5. The Services are available only to persons located in India. All or any of the Services offered through REScure health Platform may be temporarily unavailable or may not be offered to you, for various reasons which could include your medical conditions, age, geographic limitations, or prohibition / restrictions under applicable laws or Policies. Further, you should be aware that the REScure health care Associate has the right to reject provision of the Services to you at any point of time as per its policies and discretion.

6. REScure health care or the REScure health care Associate, as the case may be has the right to charge fees for the Services and the fees charged may differ from time to time. Please ascertain the fee prior to availing any Services. By availing the Services you will have consented to pay the applicable fees for the Services availed by you.

7. REScure health care may collect your bio-metric information (including fingerprint, iris scan, voice recognition and face photo) for providing the Services you require. You have the right not to provide the bio-metric information or at any time withdraw your consent given earlier for collection of the bio-metric information. The collection, storage and use of your bio-metric information by us, will be as per the [Privacy Policy], available on our Website.

8. Services such as consultation, diagnostic, physiotherapy, drug delivery require physical examination or contact with you. By availing such Services, it is deemed that you have provided your consent to the agent, employee, consultants or staff of REScure health care and / or the REScure health care Associate to visit your premises and undertake actions necessary to perform the Service.

9. We do not provide any emergency services. You are requested to dial the emergency services numbers applicable to your state / location in case of any emergency or contact your doctor.

10 This consent provided by you shall be valid for all the Services availed from REScure health care at present for only one time.

11. The Services shall be provided by the available representative of REScure health care or the REScure health care Associate, as the case may be.

12. In case of any conflict between the Terms and the Policies, the Policies shall prevail. REScure health care shall have the exclusive right to interpret the Policies and the same shall be binding on you.

13. REScure health care does not exercise any ownership, control or supervision over the REScure health care Associates or its services. All Services provided by the REScure health care Associate are subject to the policies, guidelines, registrations or other conditions implemented by the REScure health care Associate. REScure health care makes no representation or provides no warranty (express or implied) for the Services offered through the REScure health care Associate. Any claims, actions or in-actions relating to the Services availed from the REScure health care Associate should be settled inter-se between you and the REScure health care Associate.

18. In case of any dispute between you and REScure health care, in relation to the Services, the Courts of Hyderabad shall have exclusive jurisdiction.

Terms of consent for care at home

I, the undersigned, do hereby state and confirm as follows:

1. I have been explained the working, advantages and limitations thereto and I understand the same. I hereby provide my consent, authorization to REScure health care and/or service providers, to provide the REScure Home Services.

2. I state that REScure health care has answered all my questions to my satisfaction regarding the proposed Services mentioned above.

3. I have signed this consent voluntarily out of my free will and without any kind of pressure or coercion.
